ホームズは種明かしをする。靴についた六本の傷から悪天候の外出と雑な女中を、薬品のにおいや指の染み、帽子のふくらみから医業への復帰を見抜いたのだ。
表現・文法のポイント
- It is simplicity itself
- 抽象名詞＋itself で「〜そのもの」「きわめて〜」を表す。simplicity itself は「簡単そのもの」、kindness itself なら「親切そのもの」。very simple と言うより強く、ホームズらしい余裕のある言い方だ。
- the leather is scored by six almost parallel cuts
- score は動詞で「（表面に）刻み目・切り傷をつける」。得点の score とは別の意味だ。革に平行な六本の傷が刻まれている、という細部の描写から推理が組み立てられていく。
- Hence, you see, my double deduction
- hence は「それゆえに、だから」という硬い接続副詞で、後ろに名詞句だけを続けて「それゆえ〜となる」と言える。double deduction は、一つの手がかりから導いた「二重の推理」という意味だ。
- I must be dull, indeed
- dull は「鈍い、頭の働きが悪い」。後ろの if I do not pronounce him ... と合わせて、「彼を医者だと断言しなければ、私はよほど鈍いに違いない」となる。つまり、それくらい明らかだという自信の表現だ。
背景・時代
- slavey
- London slavey は、雑用を何でもこなす下働きの女中を指す当時の俗語。slave（奴隷）から来た語で、少し見下した響きがある。ホームズはそれに「靴を切り裂く悪質な」と大げさな形容をつけて皮肉っている。
- 往診医の持ち物
- iodoform（ヨードホルム）は当時の消毒薬で独特の強いにおいがあり、nitrate of silver（硝酸銀）は皮膚に黒い染みを残す。医者がシルクハットの中に聴診器をしまう習慣もあり、ホームズはそれらを手がかりにした。

"It is simplicity itself," said he; "my eyes tell me that on the inside of your left shoe, just where the firelight strikes it, the leather is scored by six almost parallel cuts. Obviously they have been caused by someone who has very carelessly scraped round the edges of the sole in order to remove crusted mud from it. Hence, you see, my double deduction that you had been out in vile weather, and that you had a particularly malignant boot-slitting specimen of the London slavey. As to your practice, if a gentleman walks into my rooms smelling of iodoform, with a black mark of nitrate of silver upon his right forefinger, and a bulge on the right side of his top-hat to show where he has secreted his stethoscope, I must be dull, indeed, if I do not pronounce him to be an active member of the medical profession."
