一八八九年の夏、結婚して診療所を開いていたワトソンは、早朝に女中の声で起こされる。パディントン駅から来た二人の男が診察室で待っているという。階段を急いで下りると、以前治療した駅員が扉を閉め、訳ありげに患者を託す。

One morning, at a little before seven o'clock, I was awakened by the maid tapping at the door to announce that two men had come from Paddington and were waiting in the consulting-room.ある朝、七時少し前に、女中が扉を叩き、パディントンから二人の男が来て診察室で待っていると告げたので、私は目を覚ました。
I dressed hurriedly, for I knew by experience that railway cases were seldom trivial, and hastened downstairs.鉄道の事故はたいてい軽くないと経験から知っていたため、私は急いで服を着て階下へ駆け下りた。
As I descended, my old ally, the guard, came out of the room and closed the door tightly behind him.階段を下りていくと、旧知の駅員が部屋から出てきて、背後の扉をしっかり閉めた。
"I've got him here," he whispered, jerking his thumb over his shoulder; "he's all right."「連れてきましたよ」と彼は肩越しに親指を向けてささやいた。「大丈夫です」
